We always welcome new patients and it would be an honor to serve you! If you would like to schedule a new patient appointment for yourself or one of your family members, please feel free to call our office at 386-668-2181 and we will gladly dedicate a special time for you to meet with Dr. Richard C. Montz for a consultation. If you would like to complete the appointment request form below, we will then contact you within one business day to confirm our appointment.

If this is an emergency, or if you are trying to cancel or change an existing appointment, please do not use this form. Contact us by phone at 386-668-2181.

If, for some reason, you cannot keep your scheduled appointment or will be delayed, please call us as soon as possible. We ask that patients kindly give us two (2) business days or 48 hours notice if they are unable to keep a reserved appointment. Our daily schedules are planned with our patients’ special needs in mind. It is our desire to provide enough time for each provider to give the very best of care to each patient throughout the day. It really helps to know ahead of time if appointments need to be changed.

In order to minimize your wait while in our office, we offer paperwork online for your convenience. We have three special forms that must be completed prior to meeting with Dr. Montz at your first dental visit to our practice. To save time at your first appointment, we would appreciate it if you would download these forms and complete them before you arrive at your first appointment. By doing this ahead of time, we will save time at your first visit. Be sure to bring these forms with you when you visit River City Dentistry.

Pre-medication Needs
Also, if your physician recommends that you be pre-medicated prior to dental treatment, it is very important for you to let us know before your scheduled appointment. We adhere to the American Heart Association guidelines for pre-medication and want to provide the precautions necessary for your specific health care needs. Should your physician require other pre-medication specifications other than what the American Heart Association recommends, we ask that your physician provide that prescription for you.

What To Expect At Your First Appointment
At the time of your first appointment with our office, each new patient can expect to have a thorough examination of their mouth and surrounding structures. During this time, you will have an opportunity to get to know your doctor and ask specific questions regarding your oral health care. Before the exam, necessary radiographs will be taken, so your doctor can best diagnose your dental needs. Along with this, an oral cancer screening exam will be given as well as a comprehensive medical history review. We believe it is very important to go through a systematic approach to guarantee the most ideal care plan for you.
